We will support you to collect evidence of your disability, usually from a doctor or another healthcare professional. We will also gather information about your life and goals. Then, an access request will be submitted to the NDIS. The National Disability Insurance Agency (NDIA – the people who run the NDIS) will review your request and make a decision about whether you can access the NDIS. The NDIA will then talk to you about your needs and approve your NDIS plan.
With this plan, you can get different supports to live comfortably and achieve your personal goals.
We can help you to:
Baptcare is an ‘NDIA Partner in the Community’, which means we work in partnership with the National Disability Insurance Agency to deliver services and support associated with the NDIS. We can provide you with your very own Local Area Coordinator.
For those accessing supports through the NDIS, a Local Area Coordinator will be your main point of contact. These services are completely free, with no obligations.
If you don’t have an NDIS Plan yet, a Local Area Coordinator can help you plan for the future and understand what services are available.
We’re always here to have a friendly chat at any point of your journey.
To get started with a Local Area Coordinator, you can call:
If you live in another state or territory, please visit this page on the official NDIS website to find the closest services to you.
If you are (or think you may be) eligible to access the National Disability Insurance Scheme, we can help with that, too. You can learn more and check your eligibility by visiting this link on the official NDIS website.
If you live in Tasmania, call us on 1800 290 555. If you live in South Australia, call us on 1800 960 595.
Your coordinator will meet with you and help identify your goals, explore if an access request to the NDIS is a suitable option and connect you to supports in your community. The meeting can happen anywhere you feel comfortable, including your home. To help you prepare for the conversation, you can fill out this easy booklet ahead of time.
If you have an NDIS plan your Coordinator will help you to use your plan. This means connecting you with services and supports in your area to help you achieve the goals in your plan. Your Coordinator will check in to see how things are going when you start using services. You can approach your Coordinator at any time with questions, concerns, or feedback. You can ask for a copy of your plan in many different accessible formats, like braille, electronic text on CD, large print text, or audio on CD, and you can have it translated into different languages, too.
We understand that your life is constantly changing. Your Coordinator will work with you to make sure your plan is updated as your situation is changing. Be sure to let your Coordinator know if you move house, or if your living situation changes. Just before the 12-month mark of using your plan, your Coordinator will talk to you about what is—and isn’t—working. Remember, you can always contact your Coordinator for support at any time.
